On Blameless RCAs

A blameless RCA does not mean accountability-free. It means:

  • Blame focuses on who made a mistake β†’ creates hiding, fear, dishonesty
  • Accountability focuses on what system change prevents the mistake β†’ creates openness, learning, improvement

The RCA asks: "What system condition allowed this mistake to be consequential?" Not "who made this mistake?"

Facilitating the RCA Meeting

  1. Establish blameless norm at the start β€” read the blameless statement aloud
  2. Timeline review first β€” get facts before analysis
  3. 5 Whys with the full team β€” stop when you hit "we chose not to invest in this"
  4. Action items must have owners in the meeting β€” never assign after the meeting

What Good Prevention Looks Like
